Consent Form

On 25th May 2018, the UK introduced its new data protection legislation, on the same day the European Union’s General Data Protection Regulation (GDPR) came into force.

These rules set the standard for how organisations should be use, hold and share your personal information and will strengthen data protection for all individuals within the European Union.

Under the new data protection laws, we require your explicit consent to use, store and share your personal data. The form below will ask for basic contact details, as well as your explicit consent for us to use your personal information to assist you with your query.

How information about you will be used

Your personal information will be stored on secure computer systems and accessed only by staff of Police and Crime Commisisoner. In order to effectively investigate your query/concern, we may need to share your information with Cleveland Police or other partner agencies. We will only use your personal information for the purpose for which it is collected.
